YOUR INFORMATION
Privacy policy.
This notice explains how information submitted through the Allianz strategy-session form is handled.
The highlighted fields below must be completed and the processing arrangements confirmed by the business before collecting real enquiries.
1. Who is responsible
The data controller is [LEGAL BUSINESS NAME / CONTROLLER], trading as Allianz. KvK registration: [KVK NUMBER]. Registered address: [BUSINESS ADDRESS].
Privacy contact: [PRIVACY EMAIL]. Responsible contact or data protection officer, if applicable: [RESPONSIBLE PERSON OR ROLE].
2. Information you provide
We receive your name, work email, phone number, company or clinic name, optional website, business type, country, goal, marketing spend, expected investment, timeline, main challenge and your consent choices.
Please do not submit patient records, health information or other sensitive personal information. This form is for business enquiries.
3. Why we use it
- To assess whether our services fit your business and respond to your request.
- To prepare for and arrange a strategy conversation when appropriate.
- To send occasional marketing insights only if you separately opt in.
- To protect the form from misuse and maintain reliable delivery.
Applicable legal bases for enquiry processing, service security and marketing must be confirmed here: [LEGAL BASES AND ANY LEGITIMATE INTERESTS]. Optional marketing uses a separate consent choice. Submitting an enquiry does not subscribe you to marketing.
4. Where your information goes
The website runs on Hostinger. Form notifications are sent through the hosting server’s email service to the business’s designated Gmail inbox. The website does not save submitted leads to public files.
Confirm the actual providers, roles, agreements, processing locations, international transfers and safeguards: [HOSTINGER, GOOGLE / GMAIL AND OTHER APPROVED SERVICES]. Access must be restricted to [AUTHORISED ROLES].
5. Retention and deletion
Enquiry records and email notifications are retained for [ENQUIRY RETENTION PERIOD AND CRITERIA]. Marketing records and consent evidence are retained for [MARKETING RETENTION PERIOD].
Security controls use a session cookie and temporary keyed rate-limit identifiers. They do not record the submitted enquiry content. Web-server logging and its retention depend on the hosting configuration: [ACCESS-LOG RETENTION AND ACCESS CONTROLS].
The responsible team must regularly review and remove expired enquiries from inboxes, archives, trash, exports and connected tools, following the agreed retention schedule.
6. Cookies and measurement
A necessary session cookie supports form security and protection against duplicate requests. No advertising tags, third-party analytics or external fonts load in this version. The page prepares anonymous interaction events in memory; those events do not contain your submitted contact details or free-text answers.
If analytics or advertising tools are introduced, this notice and the consent mechanism must be updated before those tools are enabled. Marketing email permission does not grant analytics or advertising consent.
7. Your choices and rights
Depending on the circumstances and applicable law, you can request access, correction, deletion, restriction or portability of your information, or object to processing. You can withdraw marketing consent at any time without affecting your enquiry.
Contact [PRIVACY EMAIL] to make a request or withdraw consent. You may also complain to the relevant supervisory authority, including the Dutch Autoriteit Persoonsgegevens or the Belgian Data Protection Authority.
8. Decisions and changes
Enquiry fit is reviewed by a person. The form does not make automated acceptance or rejection decisions based on your budget. This notice will be updated when the processing arrangements change.
